Use this form to amend an open complaint

An amendment is required to:

• add or remove a respondent and/or complainant to the complaint.
• add new allegations of acts, practices or omissions of unlawful discrimination, which are reasonably arguable.

Notes about this form

• Using this form is optional but helps you provide the information we need.
• To help us handle the amendment request in an effective and timely manner, please keep it to no more than 5 pages.
• Please do not provide attachments. If we need more information from you, we will let you know.
• If your amendment request is approved, a copy of this form is provided to the other party/parties to the complaint.
